As I recall, I've only seen pb kick for loss of packet flow when the player's internet connection has been lost. In other words, if I were playing in a server, and unplugged my ethernet cable, everyone else would see my character get kicked for loss of packet flow very shortly.
Of course, anything that interrupted or corrupted his/her internet connection to the server for more than, say, 30 seconds would also do this. Say, if he were running something else on his computer while he/she was gaming that also used ethernet or devices shared with ethernet inside of his/her computer. The other players can look and see that his/her ping would be 999 just before the loss of packet kick occurs. Oh, and as for the Gold edition.. shortly after it shipped, they realized it had the wrong pb, it has the pb update that should go only into a server. But, it was fixed with a later patch (a server-side patch to ignore the players with the gold edition having the wrong pb, if I recall correctly).
